The programme is tailored to your needs and may include consultations with relevant healthcare professionals. In addition, digital solutions and practical tools are used to improve your everyday life.
How the programme works
Short questionnaire
You begin by completing a questionnaire about your symptoms, your everyday life and the things that are on your mind. Your answers provide a professional starting point and ensure that the consultation is based on your situation..
Consultation with your healthcare coordinator
When you have completed the questionnaire, you book an appointment for a consultation with a Midlife specialist, who will become your dedicated coordinator. Together, you will go through your answers, discuss your needs and agree on the next steps.
A personalised programme tailored to your need
Based on your talk with the midlife specialist a personalised programme is drawn up. This may include follow‑up consultations, the involvement of relevant professionals and practical advice and tools for everyday life.
Follow‑up and adjustment
The programme can be adjusted on an ongoing basis if your needs change – so you receive the support that works best for you.
